Despite what the title says, there IS air in her lungs. She only exhales a little at the beginning. You can see her continue to exhale as she swims.
Not sure about the efficiency - clearly the goal was to complete the swim underwater, probably using technique similar to that used in freediving. When I was a kid, I could do at least 75 yards like this.
Here's another of her, completing 88 meters.
